Mission
Quality of Life Health Services, Inc. Board of Directors believes that health care is concerned with the prevention of illness and the preservation and restoration of health.

SLIDING-FEE DISCOUNT PROGRAM
Uninsured patients may be eligible to receive reduced fees through the sliding-fee discount program. Discounts range from 25% to 75% of the total charges, depending on the financial situation of the patient. Patients applying for reduced fees must provide written documentation of source of income and proof of residency at the time of service.

Patients must provide a
t least one of the following documents as proof of income:
1. Check stub with recent date 7. Bank Statements
(6 months or less) 8. VA Check Stub
2. Food Stamp Letter 9. Child Support Order
3. Social Security Letter 10. Check Stub
4. Medicaid Card
5. Unemployment Letter
6. W-2 Form (Current)

Patients must provide at least one of the following proof of residency:
1. Mail addressed to patient
2. Utility bills addressed to patient

Quality of Life accepts Medicaid, Medicare and private insurances. Discounts are available for uninsured patients upon eligibility approval.



EYE CARE SERVICES
Medicaid allows one eye exam and one pair of glasses every two years (21+ years of age) and one eye exam and one pair of glasses annually for those under 21 years of age. Medicare covers an annual eye exam.
Self-Pay fees are: $ 60.00 eye exam, $ 80.00+ glasses

PHARMACY DISCOUNT
QOLHS Pharmacies are located inside the QOLHS Health Complex (1411 Piedmont Cut-Off, Gadsden), Anniston Quality Health Care (1316 Noble Street, Anniston) and the R.O.Watts Medical Center (1020 Tuscaloosa Avenue, Gadsden). Patients can receive 25% to 75% off medications with use of the Patient Identification Card. Medicaid benefits are accepted at the QOL pharmacies. QOL pharmacies are enrolled in the following Medicare Part D Plans: Aetna, AARP, Caremark, First Health, WellCare, United Health Care and Silver Script.

PRESCRIPTION ASSISTANCE PROGRAM (PAP)
A nominal fee of $ 15.00 is required for each medication received through the Prescription Assistance Program. Eligible patients receive up to a 3-month supply of medications approved under this program. Prescriptions, income verification and tax information (if applicable) is required in determining eligibility for PAP.

PROCARE MEDICAL CENTER
No co-pay is required for Public Housing Residents and persons living within a one (1) mile radius of any Public Housing development (ProCare only).

STUDENTS OF GADSDEN CITY SCHOOLS
Students of the Gadsden City School system who are uninsured can receive care at Gadsden Family & Student Health Solutions by paying a $5.00 co-pay. Medicaid and Medicare are both accepted at the school-based site. Students and school employees with private insurance are required to pay their established co-pay amount.
